Your First Visit

For your first visit, plan to arrive 15 minutes early to allow time for initial registration and to provide information on your medical history. Please bring along any medications you are currently taking. We also need information about your insurance coverage.

Office Hours

Our office hours are 8:00am-4:00pm M-Th and 8:00am-12:00pm Friday by appointment. If you cannot keep your appointment, please give us two business days notice. We know your time is valuable and will notify you as soon as possible if your appointment time is affected by emergencies.

We have a charge for not cancelling appointments.*

*If you cancel an appointment, you must give at least 24 hours notice or your account will be subject to a $100 no show fee.

Fees

Our fees are based on our knowledge, skills, and services. Fees are charged for four primary services:

  • Consultation and office visits according to complexity of your condition.
  • If laboratory tests are performed, there is a separate charge for these.
  • If X-rays are performed there is a separate charge for these (technical and interpretation).
  • Joint procedures may be performed (aspiration and injection).

Insurance

We accept assignment on Medicare and most commercial insurance. Our office will fill out the necessary forms. Your insurance may pay all, some, or none of your bill. Any remaining balance, deductible and co-payment are the patient’s responsibility. All patients are requested to pay for their office visit on the day of service. We do not accept Medicaid.

Smoking

There are no smoking areas in our office building.

Pre-certification

Some insurances require pre-certification for special studies ordered by Rheumatology. Pre-certification may be required from your primary care provider or our office. Please notify your primary provider one week before the studies are to be performed.

Prescriptions

Prescriptions and refills are issued during regular office hours because your record is available to us for review. No routine prescriptions will be filled at night or on weekends. Please allow two business days for routine prescription refills. For refills requiring pre-authorization from your insurance company allow one week.

Confidentiality

Medical information is released only with your consent, except when required by law.

Reception Area

Please bring only one person to accompany you on your office visit. This will allow room in our reception area for all patients.
